Prehospital Management of Penetrating Trauma: The Time Is Now

This class is a primer and update on the prehospital management of penetrating trauma. Participants will review the pathophysiology of penetrating trauma from the initial injury to the progression through hemorrhagic shock to traumatic cardiac arrest. Based on this progression, prehospital interventions and time to transfer will be discussed alongside current studies supporting what makes a difference to patients and outcomes and, just as importantly, what doesn't. Participants will discuss current concepts in the management of penetrating trauma, including permissive hypotension, coagulopathy/blood product administration, tourniquets, splints, intubation, and tranexamic acid (TXA) administration. An up-to-date review of the time-dependent outcomes in penetrating trauma will also be presented, supporting rapid transport to a trauma center and minimization of prehospital interventions.
